Transportation is available:

• Monday - Saturday: 5:00 AM to 7:00 PM
• Extended Hours by Appointment

We make every effort to accommodate same day requests for transports. However, in order to ensure availability please call more than 48 hours in advance of your appointment time.

Office Hours:

• Monday - Friday: 8:00 AM to 5:00 PM
• Saturday: 8:00 AM to 1:00 PM

Messages may be left with our answering service 24 hours a day. The answering service only relays messages and does not have the authority or capacity to schedule transports. If you need to schedule or cancel a transport that will occur before the next time the office is open, the service will make every effort to contact someone to help you.

When you call please have the following information available:

• Whether the transport will be by wheelchair van or ambulance
• Name, Address and Phone Number
• Caller’s Name and Phone Number
• Date and Time of appointment
• Pick up address
• Destination address
• Payment Source

Passengers requesting wheelchair transportation to be submitted to Medicaid must register with Mitchen’s. A Certificate of Medical Necessity must be obtained from your primary care physician before any transports can be scheduled. Please have the following information ready when you call:

• Name
• Address
• Phone Number
• Medicaid Number
• Date of Birth
• Physician’s Name
• Physician’s Phone Number
• Physician’s Fax Number if possible
